Low-fat vegan diet may spur weight loss

A diet free of animal products and low in fat may help trim the waistline without the task of strict calorie watching, a new study suggests.
Researchers found that of 64 postmenopausal, overweight women, those assigned to follow a low-fat vegan diet for 14 weeks lost an average of 13 pounds, compared with a weight loss
of about 8 pounds among women who followed a standard low-cholesterol diet.
The weight loss came despite the fact that the women were given no limits on their portion sizes or daily calories — and
despite the fact that the vegan diet boosted their carbohydrate intake.
“People imagine carbohydrates to be fattening, but they are
not,” said lead study author Dr. Neal D. Barnard, an adjunct associate professor of medicine at George Washington University
in Washington, D.C.
He is also president of Physicians Committee for Responsible Medicine, a nonprofit group that advocates vegetarianism as part of preventive medicine.
The greater weight loss among women on the vegan diet may stem from specific metabolic effects, Barnard told Reuters Health.
He pointed out that the diet improved the women’s sensitivity to insulin, a hormone that ushers sugar from the blood and into cells to be used for energy. This was also accompanied by an increase in what’s known as the thermic effect of food — the amount of calories the body expends to process and store food.
The vegan diet improved women’s insulin sensitivity to a greater a degree than the comparison diet did — though the difference was not statistically significant, meaning the finding could be due to chance.
Barnard and his colleagues at George Washington and Georgetown universities report the findings in the American Journal of Medicine.
Vegan diets eschew all animal products, including dairy and
eggs, in favor of fruits and vegetables, grains, nuts and beans. Although high-protein weight-loss regimens have painted carbohydrates as the enemy, a number of studies have found that
vegetarians and vegans, who tend to eat a lot of fiber- and vitamin-rich carbohydrates, are much less likely to be overweight than meat-eaters.
Women in the current study found the vegan diet easy to follow, according to Barnard, because they were not asked to count calories or keep tabs on portion sizes. They were, however, told to avoid added oils, nuts and seeds to keep their
fat intake down.
Women in the comparison group followed a diet based on National Cholesterol Education Program guidelines, which meant restricting fat to less than 30 percent of calories and protein
to about 15 percent of calories.
Participants, who ranged in age from 44 to 73, also attended weekly meetings that included nutrition and cooking lessons.
Based on dietary records the women kept, both groups ended up reducing their calorie intake by almost 400 calories per day, on average. But those on the vegan diet lost more weight.
Despite the restrictions of going vegan, Barnard maintained
that it’s easy to take on the lifestyle. “Just eat fruits, vegetables, beans and whole grains,” he said. “Everything you’re eating is good for you.”
It is wise, he noted, to take a multivitamin, particularly to get enough vitamin B12, which is found naturally only in animal products.

Americans Prefer Employer- Selected Health Coverage

Americans would much rather choose a health plan from a set of options their employers select than to be given an employer-funded account to purchase their own plan, a new study shows.
Two-thirds of adults surveyed preferred having employers choose health plan options, with majorities of both Democrats and Republicans supporting that approach.
The study, released Thursday by the Commonwealth Fund, also revealed that people care more about being able to choose their own doctors than having a choice of health plans.
“What we’re finding is that whatever way you cut it, it looks like the type of choices that people prefer are about health-care providers, not about health-care plans, and they’re not so eager to abandon the employer-based system,” said study author Jeanne M. Lambrew, associate professor of health policy at George Washington University in Washington, D.C.
The findings call into question whether policymakers ought to be pushing health savings accounts as a way to reduce health costs and give people more control over how they spend their health-care dollars. Lambrew’s analysis suggests it may not be what most people want.
“We saw a public backlash against very closed HMOs in the 1990s,” she recalled. “We may see a similar backlash against these types of policies given the fact that people may not want them, and they prefer a type of system in which we still have employers involved in making these decisions.”
Greg Scandlen, founder of a newly formed grassroots lobbying organization called Consumers for Health Care Choices, argued that this interpretation is flawed. “This is looking at things the way politicians look at things,” he said. “They’re looking for 50-plus percent support of an idea, which is not the way markets work.”
This survey found that one-third of the people prefer choosing their own health plans, he added, “and one-third of the market is huge.”
Currently, more than 1 million Americans receive health care through a high-deductible plan paired with a health savings account, or HSA, according to a study by America’s Health Insurance Plans.
An HSA is a relatively new type of investment account. It gives people a tax-free way to save for medical expenses. Contributions to the HSA may be carried over year-to-year to pay for future medical care. Or, the account holder may withdraw funds as needed to cover current expenses, including doctor and hospital bills, prescription drugs and over-the-counter medications.
President Bush has endorsed HSAs as a way of reigning in the rising cost of health care in the United States. But critics of this approach worry that it will erode the current employer-based system of health insurance.
Using data from the Commonwealth Fund’s own biennial health insurance survey, Lambrew examined whether people really want more choice and control over their health plan options, as HSA proponents insist. The survey captures data from a nationally representative sample of 3,293 working-age adults.
The data suggest people value provider choice more highly than health plan choice. About 12 percent of those with only one health plan — offering, in other words, no other choice of plan — were likely to be dissatisfied with their health care. On the other hand, the dissatisfaction rate was more than twice as great (26 percent) among those individuals given no choice as to which provider to go to for their care.
Additionally, 67 percent of people who had no choice of where to go for their care indicated that a choice of health plans is very important. That compares with 55 of those given lots of provider choices by their plan. In contrast, just 37 percent of people restricted to one choice of health plan deemed having multiple health plan choices as very important.
“So, ironically, the people with no choice of plan didn’t really feel like it was a problem,” Lambrew said. “But the people who have very limited choice of doctors did feel like if they could just have more choices, they could get out of this box.”
Based on these new findings, Lambrew urged policymakers to “think twice” about moving away from the traditional employer-based system.
Scandlen, however, doesn’t think policymakers should bet on any single approach. “I don’t think everyone should be in an HSA plan,” he said. “Some people prefer HMOs, and they should be able to do that, too.”

The Beta Carotene-Tobacco Conundrum

Whether individuals should take a beta-carotene supplement may depend largely on whether they’ve ever smoked.
New research confirms findings from past studies that indicate people who have ever smoked — which means current and former smokers — who take beta carotene supplements are at an increased risk of tobacco-related cancers.
Conversely, lifetime non-smokers who take beta-carotene supplements enjoy the opposite effect: their cancer risk is lowered.
“We think that the most important finding [of our study] is a lower rate of cancer with increasing beta-carotene consumption in never-smokers, and the highest rate is observed with high beta-carotene in ever-smokers,” said study author Dr. Marie-Christine Boutron-Ruault, research director at the Institut national de la sante et de la recherche medicale (INSERM) in France.
“This should encourage people never to start smoking, and if unfortunately, they have smoked, to avoid taking beta-carotene supplements,” she said.
Results of the study appear in the Sept. 21 issue of the Journal of the National Cancer Institute.
Beta-carotene is a nutrient found in plants that the body uses to make vitamin A. It’s found mainly in deeply colored fruits and vegetables, such as carrots, squash, yams, peaches, apricots, spinach, collard or mustard greens, and broccoli, according to the National Cancer Institute.
Several previous studies have concluded that in smokers, beta-carotene supplements can actually increase the risk of lung cancer.
The new study looked at the incidence of lung cancer, as well as other tobacco-related cancers, such as colorectal, thyroid, ovarian and cervical cancers.
From a group of more than 60,000 French women studied for an average of 7.4 years, 700 women developed cancer known to be associated with smoking. During the study period, the women had been asked about their diets, their use of supplements and their smoking history. Only 2 percent of the women in the study used beta-carotene supplements.
For those with a history of tobacco use, rates of cancer were 368.3 per 10,000 women who had a high intake of beta-carotene, compared to 174 cases per 10,000 for those with a low intake of beta-carotene.
In women who had never smoked, those with the highest intake of beta-carotene enjoyed the lowest risk: 81.7 cases per 10,000, while those with the lowest intake had nearly 182 cases per 10,000 women, according to the study.
Boutron-Ruault said this study didn’t look at the reasons why beta-carotene can seemingly protect non-smokers but increase the cancer risk in smokers. However, she said the way cells act in the presence of high levels of beta-carotene may change when tobacco smoke is also present.
Anyone who has smoked in the past should avoid taking beta-carotene supplements, which may also be found in some multi-vitamins, the study authors pointed out. But they don’t believe that former or current smokers should be concerned about beta-carotene consumption from their diets.
“Although beta-carotene may act as a carcinogen, there is no evidence that smokers should avoid consuming beta-carotene rich foods such as fruits and vegetables, in which other components, such as vitamin C and E, may counteract a potentially deleterious interaction of beta-carotene with smoking,” the authors wrote.
Karen Collins, a registered dietician and nutrition advisor to the American Institute for Cancer Research, agreed that anyone who has ever smoked should continue to eat a varied diet that includes fruits and vegetables containing beta-carotene.
“You don’t absorb anywhere near the amount of beta-carotene from food that you do from supplements,” Collins said.
“I would strongly encourage everyone to eat an abundance of fruits and vegetables,” she added.
And for those who’ve never smoked, Collins said you can get plenty of beta -naturally by consuming the recommended five to nine servings of fruits and vegetables daily.
“Seven, eight, nine servings a day substantially lowers the risk of cancer without jumping to supplements,” she said. If you’re not getting that much from your diet, Collins added, “you need to look at supplements and their potential benefit for non-smokers.”
